Narrative

Employee was walking across a concrete pad when EE slipped on a small patch of ice and fell spraining right ankle. Accident was caused by the employee's failure to watch where EE was walking. The employee didn't violate any laws, policy, or procedures and is properly trained. Employee has a positive attitude for safety.
