## Narrative

Injured appeared to be repositioning continuous miner when the ripper head caught the injured, pinching EE between the cutter head and coal rib. Incident occurred on 5 N. Mains Section in no. 4 entry, approx. 25' inby crosscut no. 45. Injured broke 7 ribs, fractured right shoulder blade and a bone in upper neck, and punctured a lung. Injured has several bruises on chest.
