## Narrative

Pain in knee after bolting in 01/2016. IW declined treatment. 01/2017 the IW had surgery for lateral meniscus repair and debridement, patellar tendon debridement and repair of chronic patellar tendonitis. Diagnosis was a longitudinal tear of the lateral meniscus and chronic Patellar tendonitis, and early parameniscal cyst formation consistent with chronic, degenerative conditions.
