Narrative

Injured employee was climbing into a chute directly below the wash screen to apply a patch. The employee slipped while entering and struck their back against a protruding bolt or pad eye on the side wall. The employee was wearing fall protection that caused them to ""swing"" into the side wall. Employee continued to work until breathing became painful
