Narrative

Employee was helping carry a spool of cable in the 4WTG track entry when employee stepped in hole slipped and felt employee's right shoulder pop. Employee did not start missing work until 2-21-17. There were no compliance issues. It didn't involve job skills proficiency training or attitude PPE or protective devices no equipment involved.
