Narrative

The injured employee was working on clearing the plugged toilet seat on 071-04 pan feeder with a T-handle hook. As the employee dislodged a lone rock from the toilet seat, material above came down and pulled the T-handle hook downward pinching left index finger between the handle and the safety bars causing a laceration and a broken tip of the employee's left index finger.
