Narrative

On 2/14 A loader operator was climbing into loader and felt a pain/spasm in lower back. EE immediately began icing and taking OTC medication. On 2/28 ee requested to go to the clinic and was diagnosed with a sprained lumbar ligament and returned full duty. EE had a follow-up appt. On 3/10 and was prescribed physical therapy making it a reportable injury.
