## Narrative

Employee was helping center bolt in the 4WTG section under the bolter canopy when a rock fell slid down the steel and struck employee in the face causing a laceration which resulted in sutures from plastic surgery. Cause of injury was awareness. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
