Narrative

Employees were replacing a pump casing, using a hammer wrench which got stuck on a bolt. As one employee reached to try to remove the wrench, the second employee attempted to hit it with a hammer. That employee struck the other employee's thumb, injuring the nail bed. Injured employee was given restrictions.
