Narrative

The plant was removing the top section of a bucket elevator and they were unaware of another crew working on the bottom section. While removing the top section that jarred a 3""x5"" compacted piece of debris within the bucket elevator. The debris landed on worker's left hand and splitting the webbing between index finger and middle finger
