Narrative

Operator injured left forearm. The straps on a pallet of empty bags had been cut but not removed completely. EE tripped on the straps and fell forward. EE tried to catch the fall with right arm to the ground and by grabbing the handrail on the nearby steps with left hand. EE missed the railing and left forearm contacted the steps about 3"" below the elbow.
