Narrative

Using a wash down hose to clean the belly pan on 16 belt (prep plant, plant feed belt). The hose got stuck and when employee went to pull on the hose employee felt a pain in employee's lower back. Employee has history of lower back problems. No problem with mining machines, training, PPE, or compliance issues.
