Narrative

Employee was lining up a punch and die set in a Piranha Ironworker. In doing so, the set up was not aligned and the punch came into contact with the inner edge of the die. As the employee bumped the machine to recheck alignment the pressure of the punch and die contact caused a small sliver of metal to break off and strike the employee's left cheek.
