Narrative

On 5/11 an employee was working in the welding shop and felt something fall on the back of neck from a light fixture above. EE thought the light was going to fall so ee quickly jumped out of the way and right foot was caught causing to trip and land on the inside of right foot. EE felt right knee become sore. On 6/5 ee saw a doctor and put on restrictions.
