## Narrative

Employee twisted knee when climbing down from excavator.  When employee turned in FMLA/Disability paperwork on 5/31/17 for upcoming knee surgery, they had noted on the paperwork that the injury occurred at work. When employee requested time off in Feb for a Dr appointment for knee pain, they were asked it if happened at work and they responded ""No"".
