Narrative

While EE was operating shuttle car a small piece of rock came from the loading machine conveyor and struck EE in the mouth, chipping a tooth. Employee didn't violate any laws, rules, or policies and has a positive safety attitude. EE is properly trained and was wearing all required ppe.
