Narrative

Employee was outby on the E28 Longwall belt checking dollies. EE crossed the belt to move a chain off the dollies. When EE returned to the walk side of the belt, EE tripped coming off the belt and fell onto belt structure. EE felt pain in EE's left ribs. Diagnosed with a fractured rib.
