Narrative

Employee was restocking the supply tray with 6""x 6"" roof bolt plates on the Fletcher Walk-Thru roof bolter ee was operating. EE was carrying 2 bundles of roof bolt plates to the supply tray. EE lost balance. This caused the 2 bundles of roof bolt plates to smash right index finger.
