Narrative

Employee stated employee was installing roof bolts in the #4 Entry and as employee walked toward the back of the roof bolter a rock fell from between bolts and struck employee on the back of the head/neck/shoulders. The rock measured approximately 4'L x 2'W x 0"" to 2""T. This resulted in a strain and restricted work days.
