## Narrative

Injured employee received a fractured lower left leg.  Injured was bolting roof in the mine while in the man basket of the bolter.  The connection on the machine between the man-basket boom and the equipment chassis failed causing the man basket & employee to fall to the mine floor.  EMS was summoned.
