## Narrative

Roof fall was discovered in the track entry of the 6 North Haulage at 17 block. The roof fall was an estimated 16' in width x 28' long, and fell 9' above the coal seam. The area was initially bolt with a two piece combination bolt. No injuries resulted for this accident and ventilation was not impeded. A 7 man personnel carrier was trapped under the fall.
