## Narrative

Employee unhooked safety harness and started to descend a ladder secured to a portable crushing plant when their foot became entangled in the ladder about half way down (approx. 6' off the ground). They pushed off the ladder to try and avoid injury, but fell hard on their right side, fracturing 7 ribs, and collapsing a lung.
