Employee was installing a 20"" ball mill feed elbow. One threaded studs fell out because the weld broke. Employee went to the repair bay to clean up this 1"" threaded stud and to grind off the weld left over on the bolt. Employee was holding the bolt in one hand and the grinder in the other.
