Narrative

Employee was standing on scaffolding inside a tank. EE was removing a sand-blasting hose to hand it to another EE who was standing on a platform above. The hose caught on the scaffolding plank EE was standing on causing it to slide off one side of the piping it was positioned on. EE's left arm got caught on a scaffolding cross brace.
