## Narrative

EE was traveling on rubber tire personal carrier when a mechanical failure was incurred resulting in the employee losing control striking the ground with shoulder. Mechanical failure was the cause of this incident. PPE was not a factor, compliance with rules/regs was not a factor, mining equipment/mining systems was a factor, miner proficiency was not a factor.
