On 7/26/17 mine management was notified of a fall of ground at 2300 East 2000 A5 stope that went beyond the deepest support at 4:10 pm. Mine management called the MSHA hotline at 4:25 pm and was issued a verbal 103K order from MSHA at 1705 hours.
