## Narrative

Employee initially injured shoulder on 1/30/17 when ee slipped & fell while dragging water hose along a beltline. Diagnosed with a shoulder strain & rotator cuff damage. Did not violate any rules or regulations & was wearing proper PPE & is properly trained. Continued to work until 8/16/17 when ee began missing work due to surgery to repair rotator cuff.
