Narrative

EE reached down to retrieve a grate and experienced pain in back. EE reported the pain but expected the decreasing soreness to go away over time. EE did not pursue or request further treatment until 8/16/17 when EE reported increasing pain in back from the July incident. Lumbar paraspinal muscle spasms identified. Restrictions placed/PT and meds prescribed.
