## Narrative

IE's had been cleaning a sump that had never been cleaned by Redi Services. They had moved a large piece of grating for access to the sump for their hard pipe. The access was approx. 6-8"". IE stepped on the unsecured grating and fell 25' into a sump causing a laceration to  right pinky finger requiring 7 stitches.
