Narrative

Employee was moving muff cover door when it swung shut catching employee's finger resulting in a laceration. Cause of this incident was lack of employee awareness. PPE was not a factor, compliance with rules/regs was not a factor, mining systems was not a factor, miner proficiency was a factor.
