## Narrative

Employee was rigging up chain jacks on a bucket truck.  The employee hung a 3/4 ton jack to the lip of a toe board above the storage bin for temporary storage on the truck.  The employee was hanging a second jack to the same toe board when the first chain fell off and part of it struck the employee in the mouth casing a chip in the employee's front tooth.
