Employee was operating a chain hoist, lifting belt, to replace a bad roller when employee felt pain in right shoulder. EMPLOYEE HAS BEEN WORKING SINCE DATE OF ACCIDENT AND WAS TAKEN OFF WORK ON 10/11/17 DUE TO SURGERY ON BICEP PULLEY.
