## Narrative

Operating walk through bolter, installing 5' bolt to secure an already installed pull plate in a high area in St Leo. The bolt started to bend with 8""-12"" left to go in the hole, so EE started to rotate it. The up pressure, rotation and angle of bend threw the bolt wrench out and struck the left side of EE's jaw, resulting in contusions and a laceration that required 1 suture.
