## Narrative

EE was drilling hole into mine roof to install roof bolt, after drilling ee was removing drill steels when the top drill steel fell out of the roof striking ee on the finger resulting in a laceration, cause of accident was lack of concentration. PPE was not a factor, compliance with rules/regs was not a factor, mining systems was not a factor, miner proficiency was a factor.
