## Narrative

An unplanned roof fall was discovered reportable at the 69 block intersection of Main North 3 Belt. The fall was approximately 25' wide, 30' long and 18' high. The fall was most likely caused by gradual deterioration of the intersection. No problems with mining machines, PPE, training or compliance issues.
