Narrative

The roof fall occurred in the 1st south panel off the 1st main east in entry #12 between SS 41+60 and 42+20. The roof fall measured approximately 35' feet long, 18.75' feet wide, 9' to 10' feet high. This fall was partly in the examiners weekly examination route.
