Narrative

Employee was standing on the step on the side of the CM tightening a chain on a vent tube. Employee's right hand was positioned at the base of the rib drill and right elbow contacted the 'feed' lever causing the bolter head to move and pinch employee's right index & middle finger between the frame and bolter head. Had surgery to repair the tip of right index finger.
