Narrative

Employee was in the process of changing an articulation jack. Inj had already installed the cylinder end and was in the process of installing the piston end. The piston end became lodged on the ears of the main frame of the machine and inj was re-positioning the piston end when it unexpectedly shifted resulting in a partial amputation of the tip of the right middle finger.
