## Narrative

Employee was attempting to remove a drill steel that was stuck in the mine roof.  As the employee went to place another drill steel back in the chuck, the steel that was stuck, fell out striking the side of the employee's left index finger. There was no lack of job skills, training, PPE or equipment protective devices.
