Employee was dismounting Haul Truck and slipped on ladder losing footing hitting left check on handrail. Initial injury was 1/24/2018 at 14:30pm. Employee was seen by physician and released to full duty with no restriction. Employee continued to have headaches and neck pain. Employee was taken to an Occupational Therapist for further treatment and placed on restricted duty.
