## Narrative

Employee stated was attempting to install a roof bolt that was partially bent, when employee activated the rotation to line the bolt up in the drill chuck, the corner of the bearing plate struck the employee in the right forearm causing a laceration.  The employee received medical treatment (3 sutures) and returned to the mine for next scheduled shift.
