Narrative

Employee closed door to cooler room on #7 drill, felt pain in rt shoulder. MRI revealed degenerative SLAP tear, moderate tendinosis & undersurface spurring. IME noted prime motivator in impingement syndrome is aging & employee's day-to-day activities over 25 years are a substantial contributing factor to shoulder condition. On 2/13/18, employee had rt shoulder surgery.
