While roof bolting in old works, a piece of shale fell and struck employee on the head. Employee felt a sharp pain described as a ""Stinger"" in the neck and shoulder. Employee did not seek medical attention until the following day. Initially reported as a first aid only on 10/2/2017, became a lost time after surgery to repair herniated disks 2/12/2018.
