Narrative

Employee was using a hand grinder to remove a separator roller in a conveyor belt take-up. During the process EE struck self with the grinder that was equipped with a 4 ½"" cut off wheel on the top of left hand between thumb and index finger. The injury required surgery to repair the extensor tendon in the left thumb and index finger.
