Narrative

Employee was replacing an ATRS jack on a continuous miner. EE was using a chain hoist to position the replacement jack into position. The chain slipped causing the replacement jack to slip and pivot striking the employee's right hand causing a laceration to palm. Employee didn't violate any rules or regulations and has adequate training and experience. Was wearing required PPE.
