## Narrative

Another tech and myself started to disassemble the accumulator and failed to bleed off the nitrogen before starting disassembly.  I was holding the ratchet on the end cap while ee ran the ratchet.  When the end cap came to the end of its threads the pressure that was still in the accumulator blew the end cap into my hand.
