## Narrative

A mechanic was fixing a leak on the steering hose of a Komatsu 830e Haul Truck. A second employee shut the truck down so the pressure would drop off. The mechanic turned off the master switch and locked out the switch before commencing work. The pressure did not completely bleed off the hydraulic hose. When EE loosened the fitting, the hose broke free and struck EE.
