Narrative

Employee hurt shoulder while pinning. Employee had an MRI on 4/9/18 and received results on 4/11/18. The results indicated that the injury was long term illness not an acute problem. After investigation it was deemed on 4/20/18 it was an illness not injury. I have tried reporting since 4/23/18, but the MSHA website hasn't been working. MSHA was notified
