## Narrative

Employee temporarily removed grating covering a pit to clean out the pit. Employee backed up while unrolling a vacuum hose and accidentally stepped into the pit. The employee fell against I-beam over the pit, striking right shoulder. Impact from the fall caused dislocation of the right shoulder.
