Employee was climbing down the crossover ladder at 6 block when the ladder allegedly gave out. Which caused employee to twist their back and created pain in the lower lumbar area. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.
